The 26-year old has signed a four-and-a-half year deal in West London.
The initial fee could eventually rise to 26.8 million due to future incentives to be paid to the Italian club.
Fiorentina, who are currently 6th in the Serie A table, also look set to acquire Chelsea winger Mohamed Salah on loan as part of the deal.
The winger impressed at the World Cup last summer and had been linked to The Blues’ Premier League rivals Manchester United.

However, after a transfer saga that has perhaps spanned a slightly longer period than Chelsea fans expected, the Colombian eventually signed for the Premier League leaders.
The delay was due to negotiations with German club Wolfsburg over the departure of winger Andre Schurrle from Stamford Bridge, which werealso settled this evening, funding the deal for Cuadrado.
After signing on in SW6, Cuadrado told the club’s website:
I am very happy and thankful for this opportunity I’ve been given.
“This is a great club and honestly it is like a dream to join the Chelsea family and to know that the manager believes in meâ€, he added.
If Mourinho sees fit, the latest arrival at the Bridge could be available to make his debut when Chelsea travel to Villa Park in the Premier League this weekend.
